The information from this survey is used to plan for the public health, safety and economic needs of our community for the next six years.
This is a collaborative project funded and supported by Navajo County Public Health, Summit Healthcare, Little Colorado Medical Center,
Arizona Complete Health and other important community partners.
This survey represents Navajo County as a whole and we find value in a combined Community Health Assessment (CHA)
and Community Health Improvement Plan (CHIP).
